Community Supervision/Mandatory Sentence

If you were just released from State Prison, the Court, or County Jail and you were instructed to report to the Probation Department for supervision, you need to report within two working days to one of four HUBs for orientation, assessment and assignment for supervision. The HUBs are located at the following sites:

When you report to the HUB, tell the receptionist that you were just recently released from prison or jail you need your orientation instructions.  Please bring the following documents when you report for your orientation. If you do not have all of this information, have an idea or a plan as to when you will have it. If you need help getting identification documents, we may be able to provide assistance.

Birth Certificate and/or documentation of citizenship
California Driver License or California Identification Card

(or another identification with your name, picture, and signature)

Verification of residence

This can be a letter or other correspondence with your name and the address where you reside, a copy of a rental agreement, or a signed letter from your landlord verifying that the address presented is your residence.

Narcotics Offender

(H&S 11590) registration or appointment, if required.

A Copy

All document(s) you were given by the sentencing court or the Corrections Counselor regarding your release.

At the HUB you will be seen by a HUB DPO and the following will occur:

You will be provided with permanent probation instructions, which include the rules and regulations of the Probation Department for all supervised persons under Post-Release or Mandatory Supervision. This will include instructions to report to the probation officer as directed, not leave the county of Los Angeles without the permission of the probation officer or the court, and instructions to register as a narcotics, gang, and arson or sex offenders if required by law.

The probation officer will explain your conditions of supervision including all conditions required by the Probation Department or orders of the Court recorded on the sentencing minute order or court docket.

Supervised persons will also be referred to on-site Department of Public Social Service eligibility workers. While these eligibility workers will not complete applications for services on site, they will evaluate you for eligibility and refer you to the appropriate DPSS office for services if you qualify.

The HUB probation officer will also explain the payment of any court ordered restitution, fines, fees, or payments for supervision services for probationer ordered to complete Mandatory Supervision. If requested, the probation officer will refer you to a Probation Department Financial Evaluator in to determine if and how much you can afford to pay towards your financial obligation.

The probation officer will provide you with information on how and when to register as narcotics, gang, arson, or sex offender and when proof of registration is due. It is important that sex offenders register with local law enforcement with 5 days of their release from jail or they may face further prosecution for failure to register.

If required, you will be referred to co-located Department of Mental Health clinicians who will assess any need for mental health treatment services. If needed, you will be referred for these services as a condition of Post-release supervision. If these were not ordered under Mandatory Supervision, your case may be returned to court with a recommendation for the addition of treatment services as a condition of supervision.

If required, you will be referred to co-located Department of Public Health Client Engagement & Navigation Services (CENS) staff that will assess any need for Substance Abuse Treatment Services. If needed, you will be referred for these services as a condition of Post-release supervision. If these were not ordered under Mandatory Supervision, your case may be returned to court with a recommendation for the addition of treatment services as a condition of supervision.

The HUB probation officer will explain the length of supervision and eligibility for termination.

Once the HUB orientation is complete, you will be given a copy of your signed conditions of supervision, signed financial documents, and a referral to your supervision probation officer or the probation office closest to your residence. Once you report for to the supervision office, you will be assigned a supervision probation officer, who will explain the specific requirements of supervision and reporting requirements to you.
